03. Position set-up.

The position of the implants is planed:
The implant should be placed as wide apart as possible in the interforaminal area symmetrically around the middle of the symphysis.

Limitations:
1) Be at least 2 mm in front of the mental foramen to avoid the anterior loop of inferior alveolar nerve, consult the orthopantomographic radiograph for the exact size of the anterior loop.
2) The vertical height of mandible after leveling of the ridge must exceed 10 mm.
3) The distance between the center axis of placed implant must be at least 10 mm to make space for bar coping (4.8mm), soldering area) to bar (1 mm) and the retention clips (3mm). (Frialoc concept)

The posterior implants in this patient is placed posterior to the extraction sites of the lower canines to allow a bony wall between the implant and the extraction site. It is necessary to extent the leveling of alveolar ridge posteriorly to obtain the necessary width in this area.

The drilling sequence is determined by the width of the used implant and bone quality of the implant site. The bone quality can vary between preparation sites, due to different volumes of spongy bone. It is recommended to choose an implant length to enable engagement of the cortex of the inferior mandibular border. If the inferior third of the implant only rest in spongy bone or marrow spaces the resistence to mechanical loading is less.
