02. Osteotomy alveolar crest.

The lateral radiograph gives an preliminary impression of the necessary level of alveolar ridgehorizontal osteotomy to achieve a satisfactory minimal width on top of alveolar crest. Allow at least 1-2 mm marginal bone wall facial/lingual to the implant.

The space of the residual crest and mandible allowed a transgingival implant with surgical diameter 4.0 mm and length 15 mm-18mm.

In this case the height of the residual crest of the symphysis area is reduced with 3 mm, reducing the level of osteotomy toward the mental foraminal region. The osteotomy is carried out by the use of a diamond coated rotating disc and finished up with sharp straight chisel. The removed bone is preserved into sterile saline.

Prior to performing the osteotomy the middle of the symphysis area is marked with a round bur below the level of the osteotomy to ensure the symmetry of implant placement.

When the osteotomy is done the residual width of the alveolar crest is measured to 6 mm or more.
