The particles are mixed with a bone substitute if necessary up to 50% (*).

The mixture of autogeneous bone particles and the bone substitute are carefully placed between the raised antral mucosa and the bone graft. If necessary supplementary bonechips is obtained from the back of maxillary tuberosity. Finally the preserved cortical plate of the antral wall is placed on the top just beneath the antral mucosa. In case of major perforation of antral mucosa place a resorbable membran (*) on the top of the cortical plate instead.

In the end the irregularities along the bone onlay are filled with the last part of the prepared mixture (1:1 autogenous bone and the bone substitute) and the whole area is covered with a resorbable membran (*).

Careful suturing without unnecessary stretching of vestibular flap is crucial to avoid dehiscenses postoperative.
